Your psychiatric assessment is a comprehensive medical evaluation that looks at your psychological, emotional, physical, and social well-being. This visit allows our psychiatrist to determine an accurate diagnosis and make treatment recommendations to support your care moving forward.

Referral is required to access our adult psychiatry program. Once booked, we will email all required forms to the patient prior to their appointment.

Child & Adolescent Psychiatry

Our child psychiatry services support children and youth aged 5 to 17. We provide medical assessments for a range of behavioral and developmental concerns, helping families and care providers understand complex symptoms and determine the most appropriate next steps.

Please note that child psychiatry services are referral-based and require completion of all intake forms before the appointment can be confirmed. Our team will provide these forms to the parent or guardian and liaise with the referring provider as needed.

Referral Requirements

Our psychiatry program is designed as a consultation-based model to support patients through accurate diagnosis and clinical recommendations. To ensure we gather the necessary information in advance, each referral type has a specific intake process.

Adult Psychiatry Referral Requirements:

  • Valid referral from a physician or primary care provider
  • Completed adult psychiatry forms (sent by email upon booking)
  • Optional: recent lab work or supporting documentation if applicable

Child Psychiatry Referral Requirements:

  • Physician referral with presenting concerns and medical history
  • Recent lab work (within 1 month)
  • Completed parent and teacher forms, including:
    • Weiss Symptom Record II
    • Weiss Functional Impairment Rating Scale
    • Weiss Diagnostic Screen
    • SNAP-IV Teacher and Parent Rating Scales
    • CADDRA Teacher Assessment
