Appointments related to prescriptions or documentation are ideal for patients who are otherwise stable but need to maintain, update, or request something from their healthcare provider. These visits may include:

Are medical notes or forms covered by Alberta Health?

Most notes and administrative forms are not covered by provincial health insurance and may carry a small fee. This includes sick notes, employer forms, school letters, and third-party documents.

Before completing any documentation, our staff will inform you of the cost and obtain your consent. We aim to keep our pricing fair and transparent.

Can I bring a form to my regular appointment?

Absolutely. If you have an upcoming appointment for another reason and need a form or note completed, bring it with you. Let our front desk know in advance if possible so we can allocate enough time during your visit.

Our physicians will review the form, ensure it’s appropriate to complete, and help process it efficiently.

How quickly will I receive my note or form after the appointment?

Most notes and standard forms are completed during or shortly after your appointment. For more detailed or lengthy forms, we may require additional time to review medical records or collect information.

You will be informed of the timeline, and we’ll send the completed document to you via your preferred method either printed, emailed, or uploaded to your patient portal.

Do you complete forms for insurance or disability claims?

Yes, we do. Cure MD physicians can complete forms related to short-term disability, travel insurance, workplace injury, or other third-party claims, provided you have an existing patient file or supporting documentation.

In some cases, we may require a full assessment or review of previous records to complete the form accurately and ethically.

Can I request a prescription refill for someone else?

In most cases, the patient must be directly involved in the visit—either in person or virtually—to ensure safe and appropriate prescribing. This is especially important for medications with potential side effects or monitoring requirements.

If you are a caregiver or parent, exceptions may apply for dependents, provided we have their medical records and proper consent.

Do you offer virtual appointments for completing forms?

Yes, many simple forms and notes can be completed via virtual visit, especially if no physical exam is required. This can save you a trip to the clinic and speed up turnaround time.

However, if the form involves a new medical concern, physical examination, or requires specific measurements, an in-person visit will be necessary.

Can you send my completed form directly to my employer or insurance company?

Yes, with your written consent, we can securely send your form or note to a third party on your behalf. This can be done via fax, encrypted email, or secure portal upload, depending on the recipient’s requirements.
